[PHP ComboBox based on DB , Form Parameter to query and result on a grid]

Dear Php experts;

I need a sample of this  code;(Based on PHP and Mysql)

A form which contains a combobox (dropbox)  based on a mysql database, table field.

When a certain value is chosen in the combobox , this is pased to a query as a parameter in the where part.

 

The result of the query is then displayed in the form grid. Must have scroll bars.
