Php Code is Malfunctioning Url Problem HTML Involved

Alright I have a PHP code here:

preview: it checks if i have a file and returns error if fill is missing or if i placed the right name

Problem:

I Just fixed my Php code today from + into . 's now when i click the home Url Its makes a double link in the address bar i dont know if this is just for Firefox but im using FF anyone know this problem.
