Normalization Forms

I can normalize upto the 3rd normalization form but I need some help on the 4th and 5th. Can anyone assist me?

In my experience I've never found a need to go beyond 3rd.
If you are still using mysql_ functions, STOP! Use mysqli_ or PDO. The longer you leave it the more you will have to rewrite.

4th and 5th should be relegated to information theory classes.
Seriously... if people don't start reading this before posting, I'm going to consider not answering at all.

Actually I came across a project a little while ago, where the database system of the Star Trek universe was simulated (don't ask why (-: ). And we did come across 4NF and BCNF.

I must admit I have never seen a 5NF example, to date, yet. :)
