Need some help

What I'm trying to do is after the file is created in the following script, I want to open another page and pass it a few variables. I don't know how to do that.

For example, when you post something using forms, it passes the input of the forms onto the page being called.

When a user creates a file with the previous code, I want to go to a form page where the user can input some information about the file created. I need the location of the file to be passed to the next page.
