Can't get rid of slash

Need a little coding help, if you please. Forward slash showing at top of page:

Have tried a replace, eliminated all spaces, etc. Can anyone tell me how to get rid of that slash mark? THANKS!

Do you have php above your form and table?  Are you sure that all of your table, tr, and td tags start and close in the correct places?

Yes, there is PHP above, but the problem was an extra slash way down in the code, between two tags. REALLY sorry to have bothered you, stupid newbie that I am.
