forms & creating subdomains via PHP

That's actually still fairly vague in my opinion.

You should create the subdomains through your server configuration not PHP.  It all depends on what type of control panel you use.  It's more then likely Cpanel.  Just look for "sub domains" and the rest is "straight forward"

If you do not use a control pannel then it you will have to update the zone file for the domain and ether the httpd.conf or set a new rewrite rule.

So basicly you will need to study up on the [url=http://us3.php.net/manual/en/ref.filesystem.php]file system functions[/url]

Are you talking about folders inside of folders like
  Folder 1
    Folder 2

Or are you talking about
  www.site.com
  subdomain.site.com

It's not that easy to get what it is you're looking for.  I know sometimes terminology gets mixed up so I'm just trying to figure out what it is you're attempting to do here.
