Help! Need to submit files with captcha enabled

I want to create a place to receive large files from my customers. This is how it supposed to look like:



It should be able to let user upload files <30MB and with specifc extensions shown. However, I'm not able to make it fully working.


I did get file upload succesfully thru coding below, which means my server is ok.

But when I face the "restrito.php" code, I can't make it work...
Can anyone please help?
