Upload file and send to databese ? Pls Help

Hello.

first of all, i can not speak english very well. I hope, you understand me, what i mean. :)

My question;

When i selected a file (for example: image.jpg) from my pc and click to upload button;

This file send my ftp account, to the /images file and by the way, the file name insert into the database (for example: [a href=\"http://www.site.com/images/image.jpg)\" target=\"_blank\"]http://www.site.com/images/image.jpg)[/a]

What can i do ? Please help me :(

I just got this code from a tutorial website (http://www.tizag.com/phpT/fileupload.php).

I've just tested it and the upload does work.
All you need to do is add a simple database query to insert the filename into the database.
