[extracting data for a <ul> of calendar appointments]

I have a simple mysql database table that contains id, startdate, starttime, subject and description fields for a musicians gig calendar.

My goal here is to have a pulldown with the months listed where appointments have been entered. When the user selects a month, it generates something like the following:

<ul>July
<li>22 (7-10pm) at Dazzle Restaurant & Lounge (cover) - The Ken Walker Sextet</li>
<li>24 (8pm) at Bethany Lutheran Church (free) - The Aries Brass Quintet</li>
<ul>

I am so new to PHP and MYSQL that I'm at a loss as how to accomplish this.

Any help would be greatly appreciated!!!
