PHP Uploading File

I am currently trying to upload a image file to a directory on my web server and then I need to be able to store the location of this file in the database.  This way I can call the image from the database.

 

Does anyone know how I should go about doing this?

This is a script by craygo who just helped me out about 5 mins ago.Its an upload and resize script with additional code to insert the file name into a database.If you want to post any code you have I might be able to help make sense of it.
