Converting Plugin to PHP only

I've got a wordpress plugin, Comment Karma, that will work perfectly for a project I am working on, but it requires Wordpress to use it. I need to simply make it work without wordpress. I'm going to have users filling out a form of information that submits into a database. Once this information has been submitted, the "Comment Karma" bar should appear under it enabling other users to give marks good or bad.

 

Any help in doing this would be greatly appreciated. I've tried many sites but can't find any information or anyone to help. I'd even be willing to work with someone freelance if need be.
