Help with trash bin script?

I have a setup with this trash bin script:

It's basically a quick file hoster with an auto-empty feature. Not that complicated. When you upload it, there's a few form options you fill-out/choose, such as your name, your password, the file, the expiration date, and a note. It saves all this information into a single text file. I was trying to add an extra field, called type, where you select the category of the file. But I'm not very experienced with PHP. If anyone has any idea how to do this, I would appreciate it if you could point me in the right direction.
