Guidance with regards to multiple submits to same mysql table

Hi Guys,

 

I have form that has the following fields :

 

date,name,last,telephone

 

On submit it will submit to the database.

 

this becomes monotonous for the user to to fill the form and then submit and then do it again. Is there a way for me to a populate the form with however many records the user wants to fill in, and submit to the same table from that form, there fore multiple updates to the same mysql table from one form?

 

any guidance will be helpful:)
