Looking For A Form Script

Hi,
I am looking for a form script that is able to verify all the information when the SUBMIT button is pressed, has an image/code thing like when I signed into here to stop bots, send a validation email to the email entered, put all the entered information into a MySQL database along with a table for saying that is has been verified, and have it send me an email saying that someone has filled out the form.

I don't know if I will need multiple scripts or a single one is able to handle all of the needs.

I would love to learn how to do it all myself, but at the moment my free time is very limited and I am in need of the script right away.
