Two dynamic list menus in one form

I have created two dynamic forms, I would like to include them in the same form, can someone please explain how I can do this.

I'm tryin to create search with both dynamic list menus included not sure how to put everything together.

e.g. two queries for one form

or is there some way two process two forms with one button?
