Help with Array

Hello.  I have a page that I need it to forward to another page depending on how many guests you select in the drop down.  Then depending on how many people they select it will forward to a page that has ie. 5 guests names to fill out (keeping their registration info in the text areas)

 

Then what it should do is keep the data that the MAIN registration person fills out.

 

Any suggestions.  I really have tried some of the suggestions but I am not getting the right results.  So maybe someone has an easier way to help.  Thank you.
