i really need help

hi guys

this is my first post on this forum and i need help

i am developing a site on php 5 and mysql 5

i have a table with following fields

• dist_code
  
• date
  
• name
  

 

now what i want is to enter data in respective fields from a form with three fields as in mysql  database.

i want to make a check to ensure there is no duplicate entry with fields dist_code and date field if there is the user cannot add that data and if it is not then the user can post the data.

please help me
