Total Newbie help needed with forms in Expression Web2

Hello,

I am using Expressions Web2 (WYSIWYG) & I am trying to create a simple form for ordering a service ie scanning & I want to have the information (name, address, phone, email etc) along with this information: Size of photos, Resolution, & Quantity emailed to me or better yet have that info stored on the server (Linux, Apache) so I could retreive it whenever etc.

 

I would also like to have a calculator for the Quantity of photos & the Cost per photo, tax & shipping etc. on the server.  As stated, I am a total newbie so please be elementary in your response.  I appreciate your reply.  Thanks.
