using header() to download file and forcing dialog box

Below is my code to download a file.


header('Content-Type: application/force-download');
#header('Content-Type: application/octet-stream');
#header('Content-Type: application/mp3');
//header('Content-Disposition: attachment; filename='.$name);
header('Content-Disposition: attachment; filename="' . $name . '"');
readfile("$file");

 

From what I have read this should force a Save As... dialog but it does not for some reason.

 

I have tried this in FF3 Safari and Chrome.

 

Does anyone know how I can force it to display a download dialog box?
