The detailed tutorial needed to be able to do a basic database example like this would be for you to go to your local book store and get a basic php/mysql book and read it through before attempting to write any code.

 

After you store your data in a table with name, subject, and score columns -

 

name  subject  score

JOSEF SUBJECT-1 70

JOSEF SUBJECT-2 72

ARIAN SUBJECT-1 65

 

The query to retrieve any value is academic -

 

$name = 'JACOB'; // replace with the field from your form that contains the name
$subject = 'SUBJECT-3'; // replace with the field from your form that contains the subject
$query = "SELECT score FROM your_table WHERE name = '$name' AND subject = '$subject'";
