storing multiple checkboxes values into separated rows n cols (PHP)

PLEASE....i really need help.

 

in my form, i have multiple checkboxes that i want to submit using one button. at the action page,i want to manipulate the value that each of the chekckboxes holds.i want the value to be id to retrieve data from another table and then store all the information into other table.
