PHP code not sending to e-mail

Hi

 

My 2nd topic today!

 

I have a form that collects Name, address etc that I want to send back to the customer and also to me.

 

It works on a different Host (GoDaddy) but not with Freeparking.

 

Can anyone help me please.  Thank you in advance.
