How to select & send an attachment from an html form input

Hi,

 

I'm trying to set up a very simple form which will allow a user to input a name and an email address, and then choose a file from his local disk to attach to the email which will be sent when he presses the 'submit' button.

 

I've had no trouble with forms generating emails, and no trouble with a script which allows people to select files from their HD and upload to a specific directory on a webserver, but when I try to amalgamate the two and introduce snippets from other  'attachment' scripts I end up with some completely rubbish abomination which does nothing useful except save me some money at the hairdresser's.

 

I don't need any fancy file-size or file-type checking/validation. If anyone could show me a very simple script I'd happily pay them a high-quality chocolate biscuit, all the way from the UK.
