Image and text Form

OK what I'm basically trying to do is to make a form that makes this image and text appear above the image when you put your mouse over it, i want to be able to do this by form (so others are able to do this as well as me)

Also i want them to go into different categories

For example on my site i have the area it came from what type it is and the color each upload/item would fit into each of these it also has its Gem place as well for stats but that can be done by doing another upload Page 1 for each gem.
