Simple Threaded Forum Help

I found this old flatfile threaded forum script (5k), but can't get it to work. It's probably that username, etc. aren't getting posted to the script. Can't figure it out though.

 

Anyone feel like taking a look at it?

Updated download link with working version. It doesn't have nested replies though.

Here's a flatfile, threaded forum called SpiderDiscussion. Another one I can't get to work. It's not posting the form info, but I see nothing wrong with it (except poorly done HTML)..
