PHP formmail

I have this formmail.php code. its on every page of this site: http://tvdirectsite.com/

I have a HTML page with the form fields and the email address that the form goes to.

When I fill out the form and click submit, it says the form has been submitted.

The code works, the only thing that does not work is that I do not get a confirmation email when I submit the form.

What needs to be changed or added to the php code so that I will be able to get a confirmation email?

Can someone help me out with this? Any help would be appreciated.
