Multiple dates and values problem.

Hello everybody,

 

It's been a few days I'm doing  :facewall:

 

Thing is, I have a database "users" and for every user I would have enter values like on picture below

I need to enter dynamic values every day for 15 days.

You'll get it more when you see attachment. I simply have no idea how to solve this.

HELP please.
