Attaching a PDF file to an Email

I've been asked to create a change request PDF form on-line and submit via email to an approver.  The approver would be able to open the PDF with all the data that was filled out.

 

There seems to be some limitations with what Adobe Pro could handle as far as email.  They ask for user acceptance of emailing the PDF form if using the Email Submit Button.  This is unacceptable by the powers that be over the project.  I also have to use a PDF. 

 

I found that I could use a standard PDF submit button which will allow me to point to a PHP page. 

 

I'm hoping to be able to capture the PDF in it's entirety and email it with PHP.  I know how to email with PHP however, I do not know how to capture the file which is submitting the request.

 

Has anyone done anything like this? or Can you help me with some code to capture the PDF file and wrap it up into an attachment?
