hi,

my site is a computer help & troubleshooting discussion forum (i use PhpBB).

every time when users ask question we also ask some questions from them like:

 

Motherboard:

Processor/speed:

RAM/Type:

HDD/Model:

Operating system:

Browser:

Antivirus or security program:

 

and many other questions...

 

So, what i want to do is implement a feature like "Support Request template Generator" like PhpBB.com/community forums use for providing support. here: http://www.phpbb.com/support/srt/?step=1

 

by this method user can fill the form, select options of which type of help he need and then after posting their generated bb code our support team can help them better.

i already tried this modification but it's not good.

can anyone tell how to do it? it's very important.
