Created A Dynamic Form From Database But Can't Access Values

Hi I am making a clone of this webpage where the prices change automatically

I have generated this from the database but seem to be stuck as it doesn't work like the static hard coded version...don't have a clue where I am going wrong?

The idea is that the categories of radio buttons are dynamically generated from the database with the components under each heading and I have that bit...but actually using it further is posing a big problem and I am on a really tigh deadline.

 

There are actually 2 forms on the page. 1 is to change the prices and the other is to pass the chosen values to another page...neither of them work and I am stumped.

 

Any help would be greatly appreciated.
