this is all that's said in the readme file:

1. Create PHP Report tables on the file PHPREPORT.SQL.

2. Open config.inc.php and fill in the fields.

3. And enjoy it!

So basically start with going into your database management software (usually PHPMyAdmin) and import the PHPREPORT.SQL file or copy and paste the text from it and run that as a big sql command.

Upload all the files to your website in a web accessible directory.  Make sure the permissions are set right (probably 644 or 744)

The config.inc.php file is in the root application folder - fill in the values there so the program can access your database, then you should be good to go.
