Having problems Recieving email through the form I made

Hello Everyone, this is my first time to use phpfreaks, I'm actually having problems recieving the emails through the form attached to my website page. this is the website page:

Any help will be much appreciated. I really need to get that fixed as soon as possible cause there is lots of people won't be able to submit this form to us...
