Help - Undefined index: recnum

I've spent a whole day on this issue and I've read the related posts, but nothing seems to answer my problem.  I'm sure it's something simple (or stupid).

 

I've attached pdf files showing my db structure and test data, as well as the php file where the warning happens.

 

My project will collect user data and offer them a csv dump of the data for later offline analysis.  I want to use the record number (recnum in my db table) so that I may edit the data if a user spots an error.

 

You guys have been great with helping me with my project, which is nearly finished....  Thanks.
