Form File upload mysql help

I have read many many tutorials an dhave little to no experience in php but i was wondering if someone could write me a script to gather information and upload a file into mysql.

the form would go like so

Theme Name: (text field)
Artist Name: (text field small)
Description: (text field large)
Screen Shot: (upload image, jpg only.)
Theme Zip: (upload file, .zip .rar only)
Bubbles: (radio box)
Sounds: (radio box)
Icons: (radio box)

I basicly dont know how to write the script to place the form into mysql.
