DD-FORMMAILER HELP! Won't work on my designed page...

I am using (or attempting to use) Dagon's DD-FORMMAILER to create as contact form with attachment. I'm trying to link the form to a contact page (contact.php) I've already created: http://promarevents.com/VA/contact.php

 

I've kind of cut and paste the php fields from the Dagon generated form into the form that was already created. The location for the Dagon form is:

I thought it would be as easy as this but, the contact form styled for the website (contact.php) will not recognize anything put into to the required fields. It continues to give me an error message saying:

Errors!

Missing required field 'Name'

Missing required field 'LName'

Missing required field 'Phone'

Missing required field 'Email'

 

However, if I type in on the dd-formmailer.php and submit, it works fine. I hope what I wrote makes sense, but if it doesn't please try to use the form.

I am extremely new to php and am trying my best but I am lost! I'm also way behind deadline as the person I paid to do this gave me the shaft....  :'(  :-\  PLEASE HELP!
