adding new value to dropdown menu on the fly - can this be done?

I hae a dropdown list of client names that is used when entering information for a new job (like a work ticket). I would like to be able to add a new client to the list of existing clients on the fly.

This must be a common problem - but I cannot find an example when searching the site.

Can someone guide me to an existing script?

I have set up a variable in my attempts "Add new client" and when this is selected it gives one the opportunity of entering the new name - but the page has to be refreshed.
