get info in a form

Ok so I am wondering is it possible to add addition info for a form to send. I want my form to send get info to the URL looks like:

 

http://www.example.com/example.php?cat=xxx&action=xxx$select=xxx

 

my for only adds the value for select. how would I make sure get send it to look like the above?
