Could someone please help me with the syntax for my WordPress page?

 

I want to display a secure header on one page ('apply')

 

Where am I going wrong (this is in my header.php file):

<div id="wrap">
    <div id="top">
    	<div class="content">

            <div id="header">

<?php if (is_page('apply')) { ?>
