Leading zeroes is removed

I'm using dreamweaver wizard to create  database connection forms. It creates a php script and it works fine except i strips the leading zeroes from any datatype i try to put in. (char, vchar, text, int etc.)

 

I need the zeroes for storing phonenumbers.

 

I also tried changing the sprintf variable
