upload script with folder access

I havnt been here for a while but any help appreciated.- I want to be able to have an upload script that i can choose a file, then choose a folder from drop down list for the destination folder, then a username that matches that folder. I dont want to log a user in per say, just allow them to upload to their specific folder. then of course a form for me to add/delete username(folder name) and password, is it also possible to create/delete a folder via php. Is there anything like this already out there.
