Problem Submitting Form

I have a form that has a drop down list that gets company details from mysql database when the company name is selected. When the form is completed it will not submit???

 

If I remove the script that gets the data from the database i can submit the form?

 

I'm not the best at javascript, so could do with pointing in the right direction!!
