form creation

this is a pretty simple  project  but it is in 2 parts  the first part is to create a form that will insert data into a db what i am looking for is  to have 3 fields for entry

field one would be date

field 2 time

and the 3rd is a text area

which would  have something like this entered i have  taken out the  emails listed for ovious reasons

entrys into a table  where each player and email addy  are put in a new entry into data base

where each player and points are individually added to a  different table

the reason i cant use the date and time from the  top part is because we have multiple countrys and different way the dates would be entered

 

now once this has been done i have a 3 pages one is a toc list which will have a place in coding where i can change the dates on the from and to side so where i can set between august 1st and august 31st and  then when people pull up the page it will show all 1st 2nd and 3rd place players in a alphabetical list

 

2nd page would be points again using a similar date  editing spot  so it will show an accumulation of all points for that current month  that is identified

and the final  page would be a list of players and email addresses listed by player names alphabetically but if they appear more hen once then it will  only pick the latest not blank one

and this one will not have a date part on it cause it isnt used  for anything else

 

i am a director of a backgammon free tourney group and make no money on it but will pay and as well as share an accreditation  on the page so that you can take credit for your work but will pay as well just hope not 400dollars cause the moneys coming out of my pocket with no returns but gemail me a quote at sircubesalot@deepblueseagammon.com and we can work it out
