Filling a php form text field

Hey all,

 

   I'm desperately trying to find an answer to this issue. I pretty much can't go on holiday until I resolve this one damn thing.. so if you can nail it for me, you're quite literally buying me some well deserved downtime and I'll be super stoked!!
