Submitting a form with multiple rows of choices to pass on to next page

Can someone please help me figure out how this is done.  http://www.tctmerc.com/training.asp  My project is at a standstill till I figure this one out.

I need to be able to select multiple classes, and display a Contact form and the corresponding student registers for each course selected on the next page.

Then submit all that data to the DB.

Whats the problem?  it's not being inserted into the database?  Give us some code, so we can see what $vars are available and your db layout so we can get more of a picture what is going on
