help with upload class, please?

I'd love to have your help with the upload class tutorial located [a href=\"http://www.phpfreaks.com/tutorials/85/0.php\" target=\"_blank\"]here[/a]

I've looked for the zip file, but I can't seem to find it, and the link given in the commments section doesn't point to the same script
I've also searched the answer to my problem in the Tutorial Help section, the topic has been opened more than once, but there was no answer, so I've decided to post here :)

in brief, this is what I get on running the class:
