simple web album with coments

Hi im after a really simple album for my personal site. i have installed about 20different ones and they all have too much in them

here is what im after;

 

i want to beable to upload my full size  photos to a folder,  640*480 in another folder and 100*100 in another folder. then i just want to jave a scrolable thumbs list with links to the 640*480 which will link to the full sized image.

 

i would also like each image to have its own comments section using mysql

 

i dont want themes or logins or anything like that.

 

i would like to add my own authentication script to the top of the page and be able to make it look like the rest of my site.

 

any tips a a small package to get me started would be great
