add multiple data and make it generate the stored data in the same page

??? hi, need help....i need the coding

having dificulty to add multiple data in the same page

 

it is like inserting information on the client company

company id="input from keyboard"

company name = "input from the keyboard"

company address(headquarter) = "input from the keyboard"

 

company locationsite (branch):

  Branch got alot of address for this company so i need to make it so that i add  multiple branch address the data later stored and  can be viewed here on the sam page.So i can add alot of branch address for this company.

  and all the information is submit in to the database  together so that later i can call out all the information on this company.:'(..

help me on this..i have add an attachment too... thanx... :'( :'(
