[SOLVED] FILE UPLOAD not UPLOADING

I am trying to upload an image using a html form and PHP and it seems to be failing yet i am getting a 0 for array error code when i upload. The code is as followed.

well this is just more common, not really a problem.

Change

<?php $db = mysql_connect("localhost", $SQLusername, $SQLpassword) or print mysql_error (); ?>

to

<?php $db = mysql_connect("localhost", $SQLusername, $SQLpassword) or die mysql_error (); ?>

What could i be doing wrong. I am on a windows platform the code im using was used on unix hosting now its on IIS 6.0 php 5 and mysql 5 is there something in that setup i am missing?

To simplify i am having a problem with this portion I checked this portion of my code and its not working, i get the message There was an error uploading the file, please try again!
