Problem with form.

OK folkz........ Im having trouble with my forms and passing data. I can't seem to get my code to be able to pass more than one selection Or to be able to select more than one selection. I need it to be able for the user to select more than one anwser for this feild. Like "casements" and "Vinyl". Any Clues anyone?

I tried the checkbox and same result. Im at wits end........ The pets are no longer safe :|
