You ususally get it when you use an associative array without putting quotes around the key

eg

<?php
$myarray = array(
         'abc' => 1,
         'def' => 2
);

echo $myarray[abc];          // --> notice
echo $myarray['abc'];         // --> OK, no notice
?>

When you leave out the quotes the php processor assumes abc to be a constant and goes to look for the definition. If no definition is found it then assumes it to be a string with value 'abc'.

To save all that time wasting, always quote string keys.
